Your privacy is very important to both you and us. The Eye Center at Southern College of Optometry (SCO) complies with the Privacy Regulations issued by the Department of Health and Human Services in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
We understand that your health information is personal to you, and we are committed to protecting the information about you. As our patient, please know that we create medical records about your health and the services and/or items we provide to you as our patient. We need this record to provide for your care and to comply with certain legal requirements.
We are required by law to:
- Protect your health information and ensure that it is kept private;
- Provide you with a Notice of Privacy Policies and your legal rights with respect to protected health information about you; and
- Follow the conditions of the Notice that is currently in effect.

From time to time, we must communicate with other healthcare providers and agencies about you. In certain circumstances, your special approval is required. You may download a PDF copy of our record release information here. If you need a copy of your records forwarded to another provider, please download, fill in and bring this form to The Eye Center.
We regularly survey our patients to provide us with feedback regarding how we are doing. Our goal is to not only provide the best possible eye health and vision care, but we also want to provide the best patient service.
