If you are a patient or you will be assisting a patient to The Eye Center, there are some simple steps to help you prepare for your visit.
We ask that you arrive 15 minutes before your appointment, so please allow yourself enough time to arrive for your scheduled appointment. If you are unable to keep your appointment, please call and let us know in advance so we can re-schedule your visit. As a teaching facility, we ask that you plan to spend slightly more time with us than you might at your other doctors’ offices.
It is common practice for optometrists to dilate your eyes. After dilation, it may be difficult to see clearly and your eyes may be sensitive to light. If your examination requires your eyes to be dilated with drops, you may want to have someone accompany you if you will be driving or have someone drive for you.
If a patient is a minor (a child 17 years or younger), the person with the minor must be a parent or legal guardian. Anyone else with a minor must have written permission from a parent or legal guardian to make decisions about care. You may download a sample permission letter here.
Use this checklist of items to bring with you:

Bring any medical and vision insurance cards. |

If you are referred by another doctor, bring your copy of your referral. |

If you currently use glasses, contact lenses or low vision devices, bring these with you. |

Bring all current prescriptions or over-the-counter medications you take, including the dosage. |

Bring a list of your current medical doctors. Bring a list of all current health problems or conditions. |
 |
Permission slip or letter if the patient is 17 years of age or under. |
