Insurance Plans

The Eye Center accepts most insurance plans, as well as Medicare, Tennessee Medicaid and TennCare. When making an appointment, have your medical insurance or vision cards handy. See below for a list of the most common insurance plans used by our patients. If you do not see your insurance provider listed, please contact our insurance and billing representatives at 722-3250.

Patients are responsible for any co-payments, deductibles and non-covered fees. You should always check with your insurance carrier prior to your initial visit. Your insurance company is best able to make you aware of costs that you may have to pay yourself.

Bring your medical and vision insurance cards with you to your appointment. If you have been referred by another doctor, bring your referral copy, especially if your insurance carrier requires proof of the referral.

The Eye Center abides by all federal, state and local regulations regarding participation in insurance plans. We must collect all appropriate co-payments, deductibles and fees for non-covered services.
